Susan was named Vice President, Human Resources and Communications at Community Care Plan in 2014 and was promoted to the position of Vice President, Chief Human Resources Officer in 2016. Prior to joining CCP, she held Human Resources management positions at Tyco Integrated Security, LLC, Boca Raton, FL; ADT Security Services, Inc., Boca Raton, FL; Tenet Florida Health System, Fort Lauderdale, FL; and Burger King Corporation, Miami, FL.
Committed to giving back to her community, Susan volunteers for the Archdiocese of South Florida and serves as co-chairperson of the Annual Non-Profit Awards sponsored by 2-1-1 Broward.
She received a B.S. Degree from Barry University, Miami, FL; a Paralegal Certification from University of Miami; an Employee Relations Law Certification from the Institute for Implied Management and Law; and a Certification in Advanced Human Resources Management from the Society for Human Resources Management (SHRM).
Senior Vice President and Chief Financial Officer
Jason Grynbaum joins Community Care Plan as our Senior Vice President & Chief Financial Officer. Jason brings 19 years of progressive leadership experience in corporate and operational finance within healthcare and manufacturing industries, and has held various senior management roles within national managed care organizations. Prior to joining CCP, Jason was the Northeast Regional CFO at Beacon Health Options, where he lead financial planning and analysis, reporting, medical economics and cash management. Prior to that, Jason was the CFO of Peach State Health Plan, Centene Corporation's Health Plan in Georgia, where he was responsible for driving business growth by offering differentiated value and excellent customer experiences.
Jason earned a Masters of Business Administration from Carnegie Mellon University and a Bachelor of Science degree in Finance from Tulane University.
Chief Information Officer
Leon was appointed Chief Information Officer of Community Care Plan in 2015. Previously, he served as Director of Information Technology at Memorial Healthcare System for 15 years. His career highlights include serving as project lead to design, build and implement the technology infrastructure for the construction of Memorial Hospital Miramar, Memorial Data Center and the Joe DiMaggio Children’s Hospital.
Leon has demonstrated his commitment to the community as a volunteer at the West Broward High School Robotics Club and as a member of Nativity School Men’s Club. Leon received his Master of Business Administration Degree from Florida Atlantic University, Boca Raton, FL.
Vice President and Chief Business Development Officer
A 35-year veteran in the health care / health plan industry, Lupe was appointed Vice President, Government and Member Programs at Community Care Plan (formerly SFCCN) in 2007. Previously, she held management positions at Compass Health Systems, Miami, FL; Horizon Behavioral Services, Inc., Lewisville, TX; Magellan Behavioral Health, Plantation, FL; W2Credentialing, Coral Gables, FL; PsychCare, Inc, Coral Gables, FL; Aetna Health Plan, Ft. Lauderdale, FL; and Palm Springs General Hospital, Hialeah, FL.
With a reputation for excellence and outstanding performance throughout her career, Lupe was recognized as Employee of the Year during her tenures at Horizon Behavioral Health, Magellan Behavioral Health and Psych Care. She is also listed in “Who’s Who Women in Business. Lupe received an Associate Degree from Miami Dade Community College.
Senior Vice President and Chief Medical Officer
A 36-year veteran in the healthcare / health plan industry, Dr. Venereo was appointed Senior V.P. and Chief Medical Officer at Community Care Plan in 2015. Previously, he held positions as Medical Director, Sunshine Health Plan; Medical Director, Medica Healthplan; Director of Medical Affairs, Memorial Hospital West; and OB/GYN Practitioner in his own practice in Pembroke Pines, FL. He completed a fellowship in Patient Safety, and is a Diplomate of the American Board of Obstetrics and Gynecology.
Vice President, Corporate Initiatives and Process Improvement
In her position as Community Care Plan’s Vice President, Payment Operations and Project Management, Tracy Harswick is responsible for developing the strategy for payment operations within CCP, including fostering relationships with third parties utilized for these processes.
Tracy also provides oversight of CCP’s strategic project management operations, and actively participates in corporate performance improvement initiatives, to achieve the highest quality experience and results. She is the key CCP leader driving support and collaboration between departments within the company.
With 25 years of healthcare experience, Tracy has earned recognition throughout her career, including Outstanding Leadership and Exceptional Performance Award from Memorial Healthcare System (2003). She has been certified by the American Academy of Professional Coders as a Certified Professional Coder (2005). Tracy is a member of the Broward College Business and Management Programs Advisory Committee, and the National Society of Leadership and Success.
